To avail inpatient hospitalization services, you can receive treatment at any hospital * of your choice. However, you have to follow the procedures listed below to get the services.
Step 1: Take admission into the hospital.
Step 2: At the time of discharge, settle the hospital bills in full and collect all the bills, documents and reports.
Step 3: Lodge your claim with our claims team for processing and payout of Lump sum benefit. You will need to provide all the documents as listed under Documents
* Hospital / Nursing home should have a minimum of 10 in – patient beds. Please refer your policy prospectus for a complete definition. Please ensure that the hospital / Nursing home where you are contemplating treatment fulfills these criteria.
Claims Forms
For us to process your claims faster, please make sure when lodging your claim that the documents are sent as per the check list mentioned below:
- Claim Intimation Form duly filled and signed by the claimant in original
- Copy of Policy Bond
- Treating Doctors Certificate duly filled and signed in original
- Copies of Medical records, Discharge card/summary, Investigation reports, consultation papers and hospital bills.
- Attested copy of Age Proof
- Detailed medical history of the patient, from the treating Doctor (optional)
